The IRS web site is a source for information concerning both individual income and business taxes. You can find publications and tax forms available for download for both the current tax year and previous years.

Illinois state tax forms can be downloaded from the Illinois Department of Revenue web page.

If you did not get a tax refund check back, please fill out a Privacy Act Consent form and an IRS form 3911.
