MILITARY RESOURCES
Military - Active Duty
Active duty military members whose legal residence or home of record is Illinois
are welcome to contact my Illinois offices for assistance. A Privacy Act Release
Form signed by the military member is required for all inquiries with the
member's branch of service. The signed form can be returned via mail, or by fax
if the inquiry is of an immediate nature.
Military - Retired
The Defense Finance
and Accounting Service (DFAS) is the agency responsible for administering
the finances of the
Department of
Defense. All problems with retired military pay should be directed to the
DFAS Cleveland Center, while the DFAS Denver Center handles questions concerning
the Survivor Benefit Plan.
Tricare,
the health care plan for active duty and retired military members, maintains a
web site with information on its benefits.
Veterans interested in military medals and documents need to fill out
form SF-180 along with a Privacy Act Release form. |
